How to make an insurance claim?

Am I eligible for an insurance claim?

You need to have a credit protection plan with CFSL. The insurance contract namely Be Protected terms & condition have been provided to you on the date of your credit application approval.

Relating to your type of facility, an insurance claim can be raised as per below;

• Burglary and robbery affecting goods
• Death
• Permanent Disability
• Hospitalization
• Retrenchment
• Natural calamities: Fire &/or Lightning, Cyclone &/or Flood/ Tsunami 

How to make an insurance claim?

Information to be submitted to complete your insurance claim request

Applicable for both credit facility & CIM MoCredit agreement:

Type of claims & list of documents

Death 

• Claim Form - Completed and Signed
• Death Certificate
• Cause of Death Certificate

Retrenchment 

• Claim Form - Completed and Signed.
• Employer's Official Termination Letter - Inclusive of the Contract Termination date, Reason(s) of the redundancy &/Or Compensation paid by Employer.
• Document(s) to attest "Date of Entry" - Ex: Last Payslip / Work contract
• Last Payslip & Bank Statements for the last (3) months

Notes:

For CFA:

You should provide a letter confirming that you are still unemployed, and the 3 months bank statement and this every 3 months

For CIM MoCredit:

The maximum repayment for Cim MoCrédit is 6 months

Total Permanent Disability 

• Claim Form - Completed and Signed.
• Medical details from the attending physician
• Letter from Employer confirming the date of early retirement, including leave records for last 2 years
• Medical Certificate Form

Applicable for credit facility agreement only:

Type of Claims  & list of documents

Burglary and robbery affecting goods

•  Claim Form - Completed and Signed.
•  Copy of Police statement report &/Or OB Number (Inclusive of Confirmation of Violent Act &/Or Forcible Entry to or Out of the Premises)

Hospitalization 

• Claim Form - Completed and Signed.
• Medical Certificate - Inclusive of Confirmation for hospitalization being linked to an Accidental Bodily Injury.

Notes:

The Maximum repayment is 3 months.

Fire &/or Lightning 

• Claim Form - Completed and Signed.
• Copy of Police statement report (Inclusive of OB Number) &/Or Fire Services Memo.
• Technical Report (Lightning Claims ONLY) - Inclusive of "Damage Cause(s)" for Electrical items.
• Pictures (Highly recommended  but optional)

Cyclone &/or Flood/ Tsunami

• Claim Form - Completed and Signed.
• Copy of Police statement report (Inclusive of OB Number) &/Or Ministry of Social Security Memo.
• Pictures (Highly recommended  but optional)

When will I receive feedback?

You will receive feedback within 3 months, following the insurance claim request.
